Of all 88 counties in Ohio, a few had no nursing homes that met the rating scale. These counties are:

- Jefferson County
- Lawrence County

- Meigs County
- Monroe County
- Muskingum County
- Noble County
- Paulding County
- Perry County
- Ross County
- Vinton County
- Washington County
- Williams County
For the other counties listed, the overall rating percentage for each nursing home is included. The list also notes whether or not a county has nursing homes that are too new to be rated.
